RAG without magic: how to build a knowledge base that doesn't hallucinate

Chunking, embeddings, hybrid search and reranking. A practical guide to RAG that answers from sources.

RAG is not a magic way to make a model "know everything". It is a way to make it answer from selected sources and to make it possible to check where the answer came from.

In short:

  • Data and permissions first, the model second.
  • Good retrieval matters more than a bigger model.
  • Every answer should point to a source or say "I don't know".

Data before the model

Before you build a knowledge base, organize the documents. Remove duplicates, assign owners and mark versions. A model will not fix a mess in the sources.

Split knowledge by department and access level. Sales does not need to see HR documents, and legal does not need to see working notes for a project.

The layers that determine quality

ChunkingSplitting content into fragmentsFragments without context
RetrievalFinding the right fragmentsText matching only
RerankingOrder and relevanceNo noise filtering

Hybrid search

Good knowledge bases combine semantic search with classic search. Semantic search understands meaning; classic search catches names, codes and exact phrases. Only together do they deliver predictable quality.

Then comes reranking: a model assesses which fragments actually answer the question and which merely contain similar words.

Implementation checklist

  1. Collect 30–50 questions the team actually needs answers to.
  2. Define which sources are allowed for each role.
  3. Build a simple baseline: retrieval + an answer with citations.
  4. Measure relevance, completeness and the number of "I don't know" answers.
  5. Add reranking and better chunking only after measuring.
  6. Set up user feedback and source review.

When RAG is not enough

If a task requires computation, access to a transactional system or taking action, RAG alone is not enough. You need tools, permissions and approvals — in other words, an agent with clear boundaries.
